Creating a Feedback Form for Email Review Requests
This tutorial will guide you through the process of creating a feedback form necessary for sending email review requests.

First, navigate to Get More Reviews in the left sidebar and select Feedback Forms.

Next, click on Create Form and provide a name for your form.

Here you can set up the design and initial flow you'd like your customers to use. To the right, you will see the Preview.
For the widget, you can customize the funnel type and choose how you want to show feedback options such as stars, emojis, thumbs up, or review sites.

You can also add your Logo and access additional customization settings below.
In the other tabs, you can customize your feedback title and confirmation title, as well as choose to hide or show the form.

In the Distribution tab, you can customize your title and description, and adjust the toggle tabs based on your preferences.

You can also customize the title and description for the Thank You page, and change your theme by modifying the font and colors.

Additionally, you have the option to add custom CSS. Don't forget to click Save to save your changes.
